The "Application Form for Confidentiality Qualification of Weapons and Equipment Scientific Research and Production Units" (hereinafter referred to as the "Application Form") is the main documents and materials to be submitted when applying for confidentiality qualifications. Stamped with the official seal of the unit. The legal representative or main person in charge of the unit shall make a written commitment in the relevant column on the authenticity of the application conditions and application materials, ensuring the implementation of the relevant standards and the relevant responsibilities to be assumed. In addition to the Application Form, the applicant unit should also submit:

1. The original and duplicate (photocopy) of the industrial and commercial business license.

2. Organization code certificate (copy).

3. Articles of Association.

4. Financial capital verification report of the previous year. The capital verification report refers to the current or new financial capital verification certificate. The verification content shall be expressed in the form of a written report, which shall be signed or sealed by a certified public accountant, and the official seal of the accounting firm shall be affixed.

5. The property right certificate or lease contract (copy) of the scientific research and production site. The applicant unit needs to submit a certificate of property rights or a lease contract for a fixed scientific research and production site. For scientific research and production sites leased by enterprises and institutions, the lease period of the contract is generally required to be more than 3 years (inclusive).

6. The secret level certificate of the research and development project or product issued by Party A of the contract, or the letter of intent and secret level certificate of the contract signed by Party A and the unit to be undertaken for the scientific research and production tasks of weapons and equipment. The certificate of confidentiality shall be provided by Party A of the contract (the seal of the certificate is the same as the name of the unit on the seal of the contract), and it shall be reviewed by the unit's confidentiality work organization, and shall become effective after being signed and sealed by the person in charge of the unit. If Party A of the contract or the unit intended for the contract belongs to the army, it shall be signed by the person in charge of the business department above the division level and stamped with the seal of the unit. A unit that undertakes multiple secret-related contract tasks only needs to provide a secret-level certificate for one secret-related task. In principle, secret-level proof shall not involve secrets.

7. Other materials required by the National Military Industry Secrecy Certification Commission. According to the special circumstances encountered in actual work, master it in accordance with the unified requirements of the National Military Industry Confidentiality Certification Committee.

In addition, wholly state-owned enterprises do not need to provide the materials required in the above items 3 and 5, public institutions do not need to provide the materials required in the above items 1, 3 and 5, and units applying for the first-level confidentiality qualification do not need to provide the above items 5 and 6. materials required by the item.
