  2. The applicant has no occupational health and safety accident in the current year

  3. There has been no unqualified supervision and spot check by the safety supervision department within one year

  4. Satisfy the above prerequisites, fill in the Application Form for Occupational Health and Safety Management System Certification, and prepare the following main materials:

  2) Copy of business license, organization code or organization establishment approval

  3) The organization's safety production license

  4) Production process flow chart or service provision flow chart

  5) Organization chart and description of responsibilities

  8) Floor plan of production workshop

  9) Hazard list and major hazard list

  10) Occupational health and safety objectives and management plan

  11) Employee pension, medical, accident insurance payment certificate

  12) Chemical List and Material Safety Data Sheet (MSDS)

  13) Fire acceptance certificate (including dormitory)

  14) Building structure acceptance report (including dormitory)

  15) Lightning Protection Test Report

  16) Monitoring of working environment: noise, dust, etc.

  17) List of special equipment: forklifts, elevators, cranes, boilers, pressure vessels

  18) Safety test report and certificate of special equipment

  19) List of special operators: forklift operator, elevator operator, safety officer

  20) Drinking water test report (workshop and dormitory) cleaning record

  21) Indoor Air Monitoring Report
