What information do I need to apply for the product after-sales service evaluation system?

  1. The certification application, including the description of the service provision activities of the applicant organization;

  2. Copies of documents proving legal status (including: enterprise business license, public institution legal person certificate, social organization registration certificate, non-enterprise legal person registration certificate, establishment documents of party and government organs, etc.).

  3. If the service covers activities in multiple venues, a copy of the legal status certificate of each venue should be attached (if applicable);

  4. A copy of the organization code certificate (not required for the combination of three certificates);

  5. Copies of administrative license certificates, qualification certificates, compulsory certification certificates, etc. required by laws and regulations involved in the activities covered by the service;

  6. Multi-site activities and activities subcontracting;

  7, service manual and necessary procedural documents;

  8. A description that the service has been running for a certain period of time;

  9. Scope of application for certification;

  10. The applicant agrees to abide by the service certification requirements and promises to provide the information required for the review;

  11. Profile of the party under review, including the nature, name, address, legal status of the organization and relevant human and technical resources;

  12. General information about the service of the party under review and its activities;

  13. A description of the standards or other referenced documents applicable to the services to be certified;

  14. Other necessary documents related to certification review.
